Nobody knows, and Italy is the clearest case on the whole map of why. The figure everyone repeats is 15.4 cm. The largest clinical study of Italian men measured 12.5 cm stretched and never measured an erection at all, and the largest Italian dataset of any kind is 4,685 young men who measured themselves at home and reported 16.78 cm.
The claim runs 2.90 cm above the measurement, and the two do not measure the same thing: the claim says erect, the study measured stretched. There is no accepted way to convert between them.
What was actually measured in Italy?
Ponchietti 2001 is the serious one. Three thousand three hundred young men, measured by clinicians in a urology department, flaccid and stretched. The stretched mean was 12.5 cm. Not one of those 3,300 men was measured with an erection, because stretching a flaccid penis is what andrology departments do instead: it is quick, it needs no drug, and it is roughly repeatable. What it is not is a measurement of an erection. There is no accepted conversion between the two, and the four best attempts to find one disagree by about 2.8 cm, which is wider than most of the gaps this map draws between countries.
Di Mauro 2021 is the large one. Four thousand six hundred and eighty-five men, average age 19, measuring themselves at home with a ruler pressed to the pubic bone, in a warm room, and reporting 16.78 cm erect. It is a real, peer-reviewed, open-access paper, and it is the biggest Italian dataset of any kind. It is also self-measured, and it carries the fingerprint of self-measurement: a standard deviation of 2.55 cm, where every clinician-measured national study on record sits between 1.35 and 1.9 cm. Self-measurement does not only lift the average. It widens the spread, which is what happens when some men are honest and some are not.
So Italy has two good studies, 4.28 cm apart, measuring different things by different methods. The number the internet prints, 15.4 cm, is neither of them. It sits in the gap between a clinical measurement and a self-measurement, matching neither, with no method and no reference attached. That is not a compromise between two sources. It is what a number looks like when it never had a source in the first place.

What should you take from this?
If you want one number for Italy, the honest answer is that there is not one. If you want the best-evidenced statement anyone can make, it is this: Italian men measured by clinicians came out at 12.5 cm stretched, Italian men measuring themselves came out at 16.78 cm erect, and the truth is somewhere in a range wide enough to make the ranking meaningless.
The useful comparison is not with Italy anyway. It is with the clinical world figure of 13.12 cm, which was measured properly in 15,521 men and is the only number on this page that was built to be compared with. If you are inside about 3 cm of it, you are in the middle of the distribution, in Italy and everywhere else.
